Particulars of Organization, Functions & Duties

[Section-4 (1) (b) (i)]

The Employees State Insurance Scheme (E.S.I.Scheme) is an integrated multi-dimensional Health Insurance and Social Security Scheme. It is the most effective and sustainable social security measure available to the workmen employed in industrial and commercial establishments of varying sizes and nature.


  It provides comprehensive full medical care (Health Care) and reasonable economic assistance (Cash Benefit) to the beneficiaries in contingencies like hazards of sickness, maternity, disablement and death due to employment injury.


Mission / Vision:


ESI Scheme is to provide better Health Care to the Industrial Population and their family members through a net work of 4 nos. of ESI Hospitals and 41 nos. of ESI Dispensaries of the State. For providing Secondary Medical Care, Tie-up arrangements with advance medical institutions in the Country both in the public and private sector have been set up. The Central Government or the State Government is empowered to extend the provisions of ESI Act 1948 to various classes of establishments, industrial, commercial agricultural or otherwise in nature employing 10 or more persons both in organized and unorganized sector. Where the no. of coverable employees is less, panel clinics (I.M.P. System of Medical Treatment) will be implemented phase wise as per approval of ESI Corporation. Better maintenance and upkeep of Hospitals and Dispensaries, computerization of Hospitals and office management, up gradation of diagnostic services and simplification of procedure for smooth flow of benefits is also one of our primary objectives.

Brief History & Background of its Establishment:

The Labour and E.S.I Department was originally known as Labour, Employment and Housing Department. During the year 1980, the Housing wing was separated from this Department and consequent upon restructuring of Departments vide G.A Department Notification No.24622 Dt.19.12.2011 read with their Corrigendum No. 3175/Gen dt. 10.02.2012, the Employment wing has been separated from this Department. This Department has been renamed as Labour & Employees' State Insurance Department vide Office Order No.1574/LESI Dt.27.02.2012.

The E.S.I Scheme was implemented in Orisa in January’1960 with 6 Despensaries at Brajrajnagar, Rajgangpur, Choudwar, Barang, Rajabagicha and Khapuria covering a total number of 23,000 industrial workers. I initially “expanded medical care” was provided.The full medical care was introduced with effect from 1.9.1976.In the intervening years ,the ESI Scheme has been expanded by implementing in new areas. At present there are 50 Dispensaries,5 Hospitals and 2 Annexees with 297 beds and one Specialist Center spreading over 20 Districts to provide full medical benefit to 1,30,350 I.Ps/Family units.

One Ayurvedic clinic and one homeopathic clinic is also been functioning at ESI Hospital,Choudwar and at ESI Hospital,Bhubaneswar.

One 50 bedded modern Hospital at rourkela is proposed to be commissioned by ESIC very shortly.The hospital will be managed directly by the ESI Corporation at their sole cost.

It is the Nodal Department for formulating Plans, Policies and programmes for development of Labourers including child labourers (engaged under both organized and unorganized sectors) for security measures in Factories and Boilers and the social security of Industrial workers.

The ESI Act.1948 envisages an integrated social insurance scheme to provide social security to the industrial workers in certain contingencies such as sickness, maternity, temporary or permanent physical disablement and death due to employment injury resulting in loss of wages or earning capacity. The act also guarantees full medical care to the workers and their dependent family members.

Quantum, Scale and Contributory Condition:


Employees covered under the scheme are entitled to medical facilities for self and family members. They are also entitled to cash benefits in the event of sickness and employment injury which include occupational disease resulting in loss of wages or earning capacity . The insured women are entitled to maternity benefit for confinement. Where death of an insured employee occurs due to employment injury or occupational disease, the dependants are entitled to family pension.
Various benefits that the insured persons and their family members are entitled to the contributory conditions therefore, are as under :


Medical benefit :


 Medical Benefit is also available to Insured Persons who leave insurable employment on attaining the age of superannuation, after being insured for not less than 5 years.
 Medical Benefit is also available to Insured Persons who cease to be insurable employment on account of permanent disablement caused due to an employment injury.
 The Rate of contribution for superannuated/ disabled IPs is Rs. 120/-per annum payable in lump sum at the Local office for availing full Medical care for self and spouse .

Manner of Execution of Subsidy Programmes

Benefits under the Act:

The following benefits are provided to the insured persons(I.Ps.)and their family members under the provisions of ESI Act.

  1. Medical Benefit.
  2. Sickness Benefit
  3. Maternity Benefit
  4. Disablement Benefit.
  5. Dependant’s Benefit.
  6. Funeral benefit.

The First benefit i.e medical Benefit is provided by the State Government through the ESI Scheme and the remaining five benefits, which are cash benefits are provided by the ESI Corporation directly.

The Medical benefit consist of the following items:

  1. Out patient and in-patient treatment in ESI Hospitals/Dispensaries, referral Hospitals inside and outside the state hospitals.
  2. Specialist consultation.
  3. Supply of Drugs, dressing, artificial limbs, aids & appliances.
  4. Investigation facilities.
  5. Services under National Health Programmes.
  6. Payment of RCM cost of medical expenses incurred by I.P.s
  7. Medical Certification.
