Particulars of Organization, Functions & Duties
[Section-4 (1) (b) (i)]
1. Objective/ Purpose of the Public Authority:
a) Administration control of all employees of H & FW Deptt. in SDH, Dharamgarh.
b) Health Care delivery in SDH, Dharamgarh.
2. Mission/ Vision Statement of the Public Authority:
To Provide Curative, Preventive & Primitive Health Service in the Sub-division.
3. Brief History of the Public Authority and Context of its formation:
The Sub-Divisional District Health Organization under. S.D.M.O. Dharamgarh was established.
4. Duties of the Public Authority:
To provide proper Health Care to the people of the Sub-division & to maintain proper Health Administration.
5. Main Activities/ Functions of the Public Authority:
Administrative control of employees H & FW Dept.
6. List of services being provided by the Public Authority with a Brief write-up on them:
a) Medical Services (Curative):
i) SDH, Dharamgarh
ii) Block CHC's/ PHC's/ PHC (N)'s
iii) According to Block PHC's PHC (N)'s
b) Family Welfare:
i) Institutional-PPC Dharamgarh and Junagarh.

Procedure Followed in Decision Making Process
[Section-4 (1) (b) (iii)]
1. What is the procedure followed to take a decision for various matters? (A reference to Secretariat Manual and Rule of Business Manual, and other Rules/ Regulations etc. can be made).
A note sheet is prepared by dealing assistant which is put up to his supervising staff for his views than it is forwarded to the nodal officer (wing officer) after which the SDMO. Gives his approval Public decision/ sanctioned in case of matters relating to RKS the files is endorsed to Sub-Collector for approval.
2. What are the documented procedures/ laid down procedures/ Defined Criteria/ Rules to arrive at a particular decision for important matters? What are different levels through which a decision process moves?
A note sheet is prepared by dealing assistant which is put up to his supervising staff for his views than it is forwarded to the nodal officer (wing officer) after which the C..D.M.O. gives his approval. Public decision/ sanctioned in case of matters relating to ZSS & RKS the files is endorsed to Collector for approval.
3. What are the arrangements to communicate the decision to the public?
When ever the decision relates to public it is displayed in the office Notice Board & News Paper Advertisement.
4. Who are the officers at various levels whose opinions are sought for the process of decision making?
S.D.M.O.
5. Who is the Final Authority that waits the decision?
In day to day Administration, S.D.M.O. is final authority to take decision, incase of ZSS or in other committees the Collector is Final Authority.
